Welcome! We are delighted that you have selected Piedmont Internal Medicine for your primary care needs.  We are confident that you will discover great satisfaction in selecting one of our doctors as your primary care physician.  If you would like more information in selecting a physician, or would like a tour of our facilities before scheduling an actual appointment, our front office staff will be happy to assist you.

Most new patients would benefit from a complete history and physical exam session with their new doctor within the first few months of signing up for care at Piedmont Internal Medicine.  But first, we ask that our new patients schedule a brief initial visit with their new doctor before setting up a full physical exam . . . this allows the office to establish a medical record, obtain old records (if any exist and would be helpful), ensure that insurance coverage is current and accurate, and ensure that we fully participate with any specified insurance plans.  It also gives a new patient the opportunity to meet their new physician and be sure the fit is a good one before proceeding to more detailed visits.    

As it seems to be the case everywhere, the first visit to our practice starts with the collection of a host of information called demographics – names, addresses, numbers, employers, insurances, etc – and the conveying of various policies, procedures, and forms.  We ask that you arrive at least 15 minutes before your first scheduled appointment to take care of these matters in advance, so your visit with the doctor will not be cut short.  For your convenience and to speed the process, you may complete some of the paperwork in advance by printing it out ahead of time and bringing it with you already filled out and signed.  If interested in this option, see the listing of items below available for advance completion:

What to Bring to Your First Visit

Please bring the following with you to your first appointment:

O   The above 3 documents, if printed out and completed in advance
O   A copy of your current insurance card, drivers license, and SS card
O   The name and address of your previous primary care physician
O   The name and address of any specialist physicians you are seeing
O   A list of major health conditions and/or surgeries you have had
O   A list of your current medications, if any, with strengths and dosages
O   A list of your medication allergies, if any
O   Copies of any prior medical records that may be important to your care
